Collective Care Coalition
Collective Care Coalition provides trauma-informed reentry and supportive services for justice-involved individuals, returning citizens, and their families. We help participants navigate the practical, emotional, and systemic barriers associated with court involvement, reentry, and successful community stabilization.
Our mission
Successful reentry requires more than release. Justice-involved individuals and returning citizens often face interconnected barriers involving housing, employment, behavioral health, family relationships, court requirements, benefits, and access to supportive services.
Collective Care Coalition provides individualized, trauma-informed support designed to help participants navigate those barriers, remain connected to services, and build stability in the community. CCC also supports efforts that advance trauma-informed and equitable approaches to reentry and justice.

Non-legal court preparation, court reminders, appropriate accompaniment, document organization, probation and parole appointment support, post-court planning, and authorized coordination with justice-system and service partners.
CCC does not provide legal advice or legal representation.
Individualized reentry planning, identification and document assistance, benefits navigation, employment and workforce connections, housing navigation, family reunification support, referrals, and warm handoffs.
Non-clinical trauma-informed engagement, supportive listening, grief support, coping and resilience education, structured supportive activities, behavioral-health resource coordination, and referrals to qualified clinical providers when clinical treatment is needed.
CCC supportive services are non-clinical. CCC does not provide psychotherapy or clinical diagnosis.
Referral and collaboration
CCC welcomes referral and partnership opportunities with courts, probation and parole agencies, correctional and reentry programs, public defender organizations, behavioral health providers, housing organizations, employers, workforce programs, hospitals, community organizations, and government agencies.
Partnerships may include participant referrals, coordinated supportive services, reentry support, court support, resource collaboration, and other arrangements consistent with CCC's mission and organizational capacity.
