Our mission
OUR MISSION
Collective Care Coalition exists to support justice-involved individuals, returning citizens, and their families through trauma-informed, individualized, and community-centered reentry services. We help participants navigate barriers to stability by providing individualized case management, court support, reentry navigation, employment and housing navigation, family reunification support, behavioral health coordination, care coordination, and connections to community resources.
Our mission is to help individuals move from justice-system involvement toward greater stability, connection, opportunity, and successful community reintegration while promoting trauma-informed approaches throughout the systems and communities that affect their lives.
Our approach
CCC recognizes that justice-system involvement, incarceration, violence, poverty, housing instability, family separation, behavioral health needs, and other life experiences can create overlapping barriers to successful reentry.
Our approach begins with the individual. CCC works collaboratively with participants to identify needs, strengths, priorities, and barriers and to develop an individualized plan for moving forward.
Rather than expecting participants to navigate complex systems alone, CCC helps coordinate services, connect participants with appropriate resources, follow up on referrals, address emerging barriers, and support continuity of care.
Trauma-informed by design
Trauma-informed care is integrated throughout CCC's approach. We recognize that many justice-involved individuals and returning citizens have experienced trauma before, during, or after justice-system involvement.
CCC emphasizes safety, trust, dignity, collaboration, participant choice, cultural responsiveness, and empowerment. Our goal is to provide support without judgment while recognizing each participant's individual experiences, strengths, and goals.
CCC's trauma-informed supportive services are non-clinical. When participants need clinical behavioral health treatment, CCC can assist with identifying resources, coordinating referrals, and connecting individuals with appropriately qualified providers.

Our role in reentry
Successful reentry often requires coordination across multiple systems. CCC works to help bridge the gaps between participants and the organizations, agencies, providers, and community resources that can support their stability.
Depending on participant needs and appropriate authorization, this may include coordination with reentry organizations, behavioral health providers, housing and employment resources, community organizations, and justice-system partners.
Systems change and advocacy
CCC believes successful reentry requires both strong individual support and stronger systems. In addition to direct supportive services, CCC supports trauma-informed, equitable, and community-centered approaches to reentry and justice-system involvement.
Through collaboration, education, community engagement, and partnership development, CCC seeks to contribute to systems that better support successful reentry, community stability, and reduced barriers for justice-involved individuals and returning citizens.
