Finding the right resources can be challenging, especially when several needs must be addressed at the same time.
Collective Care Coalition helps justice-involved individuals and returning citizens navigate available resources and connect with organizations that may be able to assist with housing, employment, behavioral health, identification documents, benefits, legal needs, family support, and other barriers to stability.

CCC can help participants identify potential housing resources and navigate referrals. Housing availability and eligibility are determined by individual housing providers.
CCC can assist participants with employment navigation and connections to workforce resources. Employment opportunities and hiring decisions are determined by employers and workforce organizations.
Identification requirements and procedures may change. Participants should confirm current requirements with the appropriate government agency.
CCC provides non-clinical trauma-informed supportive services and behavioral health coordination. Clinical diagnosis, psychotherapy, medication management, substance-use treatment, and other clinical services must be provided by appropriately qualified providers.
Collective Care Coalition provides non-legal court support and justice navigation. CCC does not provide legal advice or legal representation.

CCC is not an emergency-response or crisis service.
If you or someone else is in immediate danger, contact 911 or the appropriate emergency service.
Individuals experiencing a behavioral health or other crisis should use an appropriate local, state, or national crisis resource.

CCC can help eligible participants identify needs, organize priorities, explore available resources, coordinate referrals, and develop an individualized plan for moving forward.
