Finding the right resources can be challenging, especially when several needs must be addressed at the same time.

Collective Care Coalition helps justice-involved individuals and returning citizens navigate available resources and connect with organizations that may be able to assist with housing, employment, behavioral health, identification documents, benefits, legal needs, family support, and other barriers to stability.

CCC provides non-clinical trauma-informed supportive services and behavioral health coordination. Clinical diagnosis, psychotherapy, medication management, substance-use treatment, and other clinical services must be provided by appropriately qualified providers.
