REQUEST SUPPORT
Are you justice-involved, returning to the community, or navigating barriers related to reentry? CCC provides individualized case management, court support, reentry navigation, employment and housing navigation, family reunification support, behavioral health coordination, care coordination, and trauma-informed supportive services based on individual needs and organizational capacity.
Submitting this form does not guarantee acceptance into CCC services. CCC will review the inquiry based on individual needs, scope of services, and organizational capacity.
REFER A PARTICIPANT
Professionals and organizations may submit an initial referral inquiry for an individual who may benefit from CCC services. CCC welcomes appropriate referrals involving case management, court support, reentry navigation, housing and employment navigation, family reunification support, behavioral health coordination, care coordination, and trauma-informed supportive services.
Submission of a referral does not guarantee enrollment or acceptance of services. CCC reviews referrals based on participant needs, scope of services, organizational capacity, and appropriate consent.
PARTNER WITH CCC
CCC welcomes conversations with organizations and professionals interested in developing referral relationships, resource collaborations, coordinated services, or other mission-aligned opportunities supporting justice-involved individuals and returning citizens.
Submitting a partnership inquiry does not establish a formal partnership, contract, referral agreement, or affiliation with CCC.
