REQUEST SUPPORT

Are you justice-involved, returning to the community, or navigating barriers related to reentry? CCC provides individualized case management, court support, reentry navigation, employment and housing navigation, family reunification support, behavioral health coordination, care coordination, and trauma-informed supportive services based on individual needs and organizational capacity.

Information submitted to CCC will be handled according to applicable organizational privacy and confidentiality practices. Additional authorization may be required before CCC communicates with outside organizations regarding an individual's services.

Submitting this form does not guarantee acceptance into CCC services. CCC will review the inquiry based on individual needs, scope of services, and organizational capacity.

REFER A PARTICIPANT

Professionals and organizations may submit an initial referral inquiry for an individual who may benefit from CCC services. CCC welcomes appropriate referrals involving case management, court support, reentry navigation, housing and employment navigation, family reunification support, behavioral health coordination, care coordination, and trauma-informed supportive services.

PARTNER WITH CCC

CCC welcomes conversations with organizations and professionals interested in developing referral relationships, resource collaborations, coordinated services, or other mission-aligned opportunities supporting justice-involved individuals and returning citizens.
