Collective Care Coalition works to build connections among justice-involved individuals, returning citizens, families, community organizations, service providers, employers, housing resources, justice-system stakeholders, and others committed to strengthening pathways to stability.

Collective Care Coalition welcomes conversations with organizations and professionals interested in strengthening support for justice-involved individuals and returning citizens.

CCC is interested in developing referral relationships, coordinated service arrangements, resource partnerships, and other mission-aligned collaborations that expand access to practical support and reduce gaps between systems.

REFER A PARTICIPANT

CCC welcomes appropriate referrals for justice-involved individuals and returning citizens who may benefit from individualized case management, court support, reentry navigation, employment or housing navigation, family reunification support, behavioral health coordination, care coordination, or trauma-informed supportive services.

Referral acceptance and service planning depend on the individual's needs, CCC's scope of services, organizational capacity, and appropriate participant consent.

Stable housing remains one of the most significant barriers many returning citizens face.

CCC welcomes relationships with housing providers, transitional and supportive-housing programs, property partners, reentry organizations, and community resources that may help participants identify safe and appropriate housing options.

COORDINATE SUPPORT

CCC recognizes that successful reentry may involve behavioral health, healthcare, substance-use treatment, family services, and other specialized supports.

CCC welcomes relationships with qualified providers and community organizations that can help strengthen continuity of care and expand appropriate referral options.

CCC does not represent referral relationships as clinical treatment provided by CCC. Clinical services must be delivered by appropriately qualified or credentialed providers.

VOLUNTEER WITH CCC

Volunteers can help strengthen CCC's community presence and organizational capacity.

Volunteer opportunities may develop in areas such as community outreach, resource development, events, administrative support, program support, and other mission-aligned activities based on CCC's current needs.

Volunteer roles involving direct participant contact may require additional screening, orientation, training, confidentiality requirements, and supervision. Returning citizens are welcomed!
